Individuals and their families will need to be supported and allowed time to adjust and become accustomed to any form of transition and change. Effective support during this time can help the individual cope with change more easily.

Carers should consider the impact the change may have upon the individual and their family and the range of emotions this may cause, including;

  • fear
  • anger
  • sense of loss
  • confusion.

The carer should also consider whether the planned change is appropriate for the individual and their family. It is important to involve them in all decisions made about their care and support.

The individual should be helped to prepare for the change ahead. This could either be through verbal or visual prompts, or through the use of a countdown calendar.

They will need to be able to ask questions and have those answered clearly in a way that they can understand. Patience and perseverance will be required if an individual or their family is particularly worried about, or resistant to change.

The impact of change can be positive or negative.

Positive

Respite care can have a positive impact on the family as they can get on with their daily lives without feelings of guilt knowing that their loved one’s needs are being taken care of.

Companionship may have been something that was lacking in an individual’s life. If they live alone, they may go for days without human contact. Having home-based care can alleviate feelings of isolation and loneliness.

The individual will be offered the opportunity of support in order for them to engage in meaningful activities that can promote self-esteem and mental well-being.

Relationships between the individual and their family and/or partner can improve when there is less responsibility on the family to care for the individual.

Negative

There could be financial burden on the family for the cost of services that are required by the individual.

The individual may be in denial of the changes that are occurring feeling like they are losing control of their lives, become depressed over losing the ability to do certain things, feel there are failing their family due to not being able to carry out their role as a parent/partner etc.

Efallai fod cwmnïaeth wedi bod yn rhywbeth oedd yn ddiffygiol ym mywyd unigolyn. Os ydynt yn byw ar eu pen eu hunain, gallant fynd am ddyddiau heb gyswllt dynol. Gall cael gofal yn y cartref leihau teimladau o arwahanrwydd ac unigrwydd.

In order to help an individual maintain their sense of identity it is important to always recognise and value their unique style, life story and preferences. Their religion, culture and sexuality should also be considered when planning their support.

Just because an individual lacks the ability to do certain things for themselves doesn’t mean that they don’t feel a sense of frustration, confusion or embarrassment about having another do these things for them. A carer should be sensitive to the individual’s feelings in order to minimise the risk of damaging the individual’s sense of self and making them feel that their life has no purpose. They can promote the individual’s self-identity through focusing on their positive attributes and supporting how they wish to present themselves.

Self-identity is the outcome of social contact and feedback from those around them. It is important that the individual is given opportunities to be involved in meaningful activities with those in their community.

How a carer responds to the individual in their care is key to positive relationships. If the carer has good body language, eye contact and active listening skills, it’s more likely to have a positive impact.

A carer should also be mindful of the individual’s preferences in relation to their spiritual, cultural and religious beliefs and life choices such a gender identity and sexuality.

It is important that a carer is non-judgemental and not only accepts, but supports, the way an individual and their family choose to live their lives.

It is important that both the individual and their family/carer are involved in all planning of services so that they understand the type of services and support they will receive. It is during these planning meetings that the individual and their family will be able to ask questions and make their needs and preferences clear.

This is an important part of ensuring that the support provided fits the needs of the individual and also ensures that everyone understands the extent and limitations of the support.

There is a legal requirement under the Social Services and Well-being (Wales) Act 2014 to plan and support individuals to plan for their future care where practicable.

Mae'n bwysig bod yr unigolyn a'i deulu/gofalwr yn cael eu cynnwys wrth gynllunio'r holl wasanaethau fel eu bod yn deall y math o wasanaethau a chymorth y byddant yn eu derbyn. Yn ystod y cyfarfodydd cynllunio hyn bydd yr unigolyn a'i deulu yn gallu gofyn cwestiynau a sicrhau bod eu hanghenion a'u dewisiadau yn glir.

Mae yna ofyniad cyfreithiol yn Neddf Gwasanaethau Cymdeithasol a Llesiant (Cymru) 2014 i gynllunio a rhoi cymorth i unigolion gynllunio am eu gofal yn y dyfodol lle bo’n ymarferol.
