This is a legal framework that brings together and modernises social services law.

Its purpose is to impose duties on local authorities, health boards and Welsh ministers that require them to work to promote the well-being of those who need care and support.

The principles of the Act include:

Partnership and integration – The Act states that local authorities, health boards and NHS trusts must work closely to ensure better integration of health and social care and that their resources are used effectively to improve the well-being of individuals needing care and support.

Person-centred – The Act ensures assessments are carried out in the best way to meet an individual's needs. By working in partnership and putting individuals at the centre, professionals can focus on what matters to them and how they can use their own strengths and resources. Care planning, safeguarding and financial assessments are also considered. Individuals are given a stronger voice and control over services they receive.

Prevention – The Act states that local authorities will arrange more preventative services to support people before their needs become critical. They are also required to provide bilingual information, advice and assistance services.

Well-being – The Act supports people who have care and support needs to achieve well-being through having their rights listened to, being physically, mentally and emotionally happy, protected from abuse, harm and neglect, having education, training, sports and play opportunities and positive relationships with family and friends.

Ei diben yw gosod dyletswyddau ar awdurdodau lleol, byrddau iechyd a gweinidogion Cymru sy'n ei gwneud yn ofynnol iddynt weithio i hybu llesiant y rheini y mae angen gofal a chymorth arnynt.

Mae egwyddorion y Ddeddf yn cynnwys y canlynol:

Partneriaeth ac integreiddio – Mae'r Ddeddf yn datgan bod yn rhaid i awdurdodau lleol, byrddau iechyd ac ymddiriedolaethau'r GIG weithio'n agos er mwyn sicrhau bod gwasanaethau iechyd a gofal cymdeithasol wedi'u hintegreiddio'n well ac y caiff eu hadnoddau eu defnyddio'n effeithiol i wella llesiant unigolion y mae angen gofal a chymorth arnynt.

Person-ganolog – Mae'r Ddeddf yn sicrhau y caiff asesiadau eu cynnal yn y ffordd orau i ddiwallu anghenion unigolyn. Drwy weithio mewn partneriaeth a sicrhau bod unigolion yn greiddiol i'r broses, gall gweithwyr proffesiynol ganolbwyntio ar beth sy'n bwysig iddynt a sut y gallant ddefnyddio eu cryfderau a'u hadnoddau personol. Rhoddir ystyriaeth hefyd i gynllunio gofal, diogelu ac asesiadau ariannol. Rhoddir llais cryfach i unigolion a mwy o reolaeth dros y gwasanaethau a gânt.

Atal – Mae'r Ddeddf yn nodi y bydd awdurdodau lleol yn trefnu gwasanaethau mwy ataliol sy'n helpu pobl cyn i'w hanghenion fynd yn rhai critigol. Mae hefyd yn ofynnol iddynt ddarparu gwybodaeth, cyngor a gwasanaethau cymorth dwyieithog.

Llesiant – Mae'r Ddeddf yn helpu pobl ag anghenion gofal a chymorth i sicrhau llesiant drwy sicrhau y gwrandewir ar eu hawliau, eu bod yn hapus yn gorfforol, yn feddyliol ac yn emosiynol, y cânt eu hamddiffyn rhag cael eu cam-drin, rhag niwed a rhag esgeulustod, y cânt addysg, hyfforddiant, cyfleoedd i wneud chwaraeon a chyfleoedd chwarae a chydberthnasau cadarnhaol â theulu a ffrindiau.
