Multi-disciplinary teams are a group of health care workers who are members of different disciplines/professions that work in various agencies.
Grŵp o weithwyr gofal iechyd yw timau amlddisgyblaethol sy'n aelodau o ddisgyblaethau/proffesiynau gwahanol sy'n gweithio mewn asiantaethau amrywiol.
They play a key role in ensuring that all aspects of service users’ needs are properly considered, so that appropriate advice on treatment can be given. This is known as the holistic approach.
Anyone requiring care must have an assessment of need. Once their needs have been identified, the team will work with the service user to decide on the best care to be given and this will provide a seamless service, which is based on ‘best practice' and be the most beneficial to the service user.
