New treatment fund

Cronfa triniaethau newydd

The Welsh Government has committed £80 million to speed up access to the latest medicines.

The fund will provide additional support of £16 million annually to help health boards in Wales speed up access to medicines recommended by the National Institute for Health and Care Excellence (NICE) and the All Wales Medicines Strategy Group.

Under the new system, all health boards in Wales will be required to make a NICE or AWMSG recommended medicine available no later than two months from the date the final guidance is published.

Mae Llywodraeth Cymru wedi ymrwymo £80 miliwn i gyflymu mynediad i'r meddyginiaethau diweddaraf.

Bydd y gronfa yn rhoi cymorth ychwanegol o £16 miliwn y flwyddyn er mwyn helpu byrddau iechyd yng Nghymru i gyflymu mynediad i'r meddyginiaethau a argymhellir gan y Sefydliad Cenedlaethol dros Ragoriaeth mewn Iechyd a Gofal (NICE) a Grŵp Strategaeth Meddyginiaethau Cymru Gyfan.

O dan y system newydd, bydd yn ofynnol i bob bwrdd iechyd yng Nghymru sicrhau bod meddyginiaethau a argymhellir gan NICE neu AWMSG ar gael dau fis o'r dyddiad y caiff y canllawiau terfynol eu cyhoeddi fan bellaf.

Building a Brighter Future: Early years and childcare plan

Adeiladu Dyfodol Mwy Disglair: Cynllun y blynyddoedd cynnar a gofal plant

This plan sets out the Welsh Government’s commitment to improve the life chances and outcomes of all children in Wales.

There are seven Sections to the plan:

  • Section 1 explains why investing in the early years is so important
  • Section 2 focuses on children’s health and well-being
  • Section 3 looks at the importance of strong and positive families
  • Section 4’s theme is high-quality early education and childcare
  • Section 5 addresses the importance of a good primary education
  • Section 6 sets out how they expect to raise standards
  • Section 7 clarifies the roles and responsibilities for implementation.

Mae'r cynllun hwn yn nodi ymrwymiad Llywodraeth Cymru i wella cyfleoedd bywyd a chanlyniadau pob plentyn yng Nghymru.

Mae saith Adran i'r cynllun:

  • mae Adran 1 yn egluro pam y mae buddsoddi yn y blynyddoedd cynnar mor bwysig
  • mae Adran 2 yn canolbwyntio ar iechyd a llesiant plant

Healthy Child Wales

Plant Iach Cymru

The Healthy Child Wales Programme is a universal health programme for all families with 0 – 7 year old children.

It includes a consistent range of evidence based preventative and early intervention measures, and advice and guidance to support parenting and healthy lifestyle choices.

The HCWP sets out what planned contacts children and their families can expect from their health boards from maternity service handover to the first years of schooling. These universal contacts cover three areas of intervention:

  • screening
  • immunisation
  • monitoring and supporting child development.

Mae Rhaglen Plant Iach Cymru yn rhaglen iechyd gyffredinol ar gyfer pob teulu â phlant 0 – 7 oed.

Mae'n cynnwys amrywiaeth gyson o fesurau ataliol ac ymyrraeth gynnar sy'n seiliedig ar dystiolaeth, a chyngor ac arweiniad er mwyn cefnogi dewisiadau rhianta a ffordd iach o fyw.

Mae'r rhaglen yn nodi pa gysylltiadau a gynlluniwyd y gall plant a'u teuluoedd eu disgwyl gan eu byrddau iechyd o gyfnod trosglwyddo'r gwasanaeth mamolaeth i'r blynyddoedd cyntaf o addysg. Mae'r cysylltiadau cyffredinol hyn yn cwmpasu tri maes o ymyrraeth:

  • sgrinio
  • imiwneiddiad
  • monitro a chefnogi datblygiad plant.

Flying start

Dechrau'n Deg

Flying Start is part of the Welsh Government’s early years programme for families with children under 4 years of age living in disadvantaged areas of Wales.

There are 4 key elements to Flying Start.

Quality, part-time childcare for 2-3 year olds.

Flying Start offers up to two and a half hours of child care a day, five days a week, to parents of all eligible 2-3 year olds.

An enhanced health visiting service.

The primary function of the Flying Start health visitor is to support the family in the home, assessing both the child and the family (in terms of high, medium and low risk).

Access to parenting programmes.

Parenting programmes are offered covering the three themes:

  • perinatal and support in the early years
  • early intervention approaches to supporting vulnerable parents
  • programmes to support parents in positive parenting.

Speech, language and communication.

Every family in a Flying Start area has ongoing access to an appropriate language and play group. From this, a more targeted approach based on assessment and referral can be taken where there is evidence of additional need.

Prif swyddogaeth ymwelydd iechyd Dechrau'n Deg yw cefnogi'r teulu yn y cartref, gan asesu'r plentyn a'r teulu (o ran risg uchel, canolig ac isel).

Cael y cyfle i fynychu rhaglenni rhianta.

Cynigir rhaglenni rhianta sy'n cwmpasu'r tair thema:

  • gofal a chymorth amenedigol yn y blynyddoedd cynnar

Welsh Network of Healthy Schools Schemes

Rhwydwaith Cymru o Gynlluniau Ysgolion Iach

Welsh Network of Healthy Schools Schemes actively promotes, protects and embeds the physical, mental and social health and well-being of its community through positive action.

The ‘Healthy School’ is one which takes responsibility for maintaining and promoting the health of all who ‘learn, work, play and live’ within it not only by formally teaching pupils about how to lead healthy lives but by enabling pupils and staff to take control over aspects of the school environment which influence their health.

Mae Rhwydwaith Cymru o Gynlluniau Ysgolion Iach yn mynd ati i hybu, amddiffyn ac ymgorffori iechyd a llesiant corfforol, meddyliol a chymdeithasol ei gymuned drwy gamau gweithredu cadarnhaol.

Mae'r 'Ysgol Iach' yn un sy'n cymryd cyfrifoldeb am gynnal a hybu iechyd pawb sy'n 'dysgu, gweithio, chwarae a byw' ynddi drwy addysgu disgyblion yn ffurfiol ynghylch sut i fyw bywydau iach yn ogystal â galluogi'r disgyblion a'r staff i gymryd rheolaeth dros agweddau ar amgylchedd yr ysgol sy'n dylanwadu ar eu hiechyd.

A Healthier Wales

Cymru Iachach

The Welsh Government’s vision is that everyone in Wales should have longer, healthier and happier lives, able to remain active and independent, in their own homes, for as long as possible.

The plan focuses on health and well-being, and on preventing illness by providing:

  • A whole system ‘wellness’ approach to health and social care which aims to support and anticipate health needs, to prevent illness, and to reduce the impact of poor health.
  • Services and support that deliver the same high quality of care, and achieve more equal health outcomes, for everyone in Wales. This will improve the physical and mental well-being of all, throughout their lives, from birth to a dignified end.
  • Access to a range of services which are made seamless, and delivered as close to home as possible.
  • Services designed around the individual and around groups of individuals, based on their unique needs and what matters to them, as well as quality and safety outcomes.
  • Hospital services designed to reduce the time spent in hospital, and to speed up recovery. The shift in resources to the community will mean that when hospital-based care is needed, it can be accessed more quickly.

Gweledigaeth Llywodraeth Cymru yw y dylai pawb yng Nghymru gael bywyd hirach, iachach a hapusach, gallu parhau i fod yn egnïol ac yn annibynnol, yn eu cartrefi eu hunain, am gyhyd â phosibl.

Free prescriptions

Presgripsiynau am ddim

On April 1st 2007 the NHS prescription charge was abolished for individuals in Wales.

All patients registered with a Welsh GP, who get their prescriptions from a Welsh pharmacist, are entitled to free prescriptions.

Prior to the introduction of free prescriptions in Wales, prescriptions were only free for those individuals under 25 and over 60 or who had certain medical conditions.

The system was unfair because individuals with certain conditions such as asthma, heart disease and high blood pressure were not entitled to free prescriptions whilst individuals with conditions such as diabetes were.

The new policy ensures fairness for all and the free access to medicine ensures individuals are treated early and hospital admissions are avoided.

Ar 1 Ebrill 2007, cafodd y tâl am bresgripsiwn y GIG ei ddiddymu i unigolion yng Nghymru.

Mae gan bob claf sydd wedi'i gofrestru â meddyg teulu yng Nghymru, sy'n cael ei bresgripsiynau gan fferyllfa yng Nghymru, hawl i gael presgripsiynau am ddim.

Cyn cyflwyno presgripsiynau am ddim yng Nghymru, dim ond yr unigolion hynny dan 25 oed a thros 60 oed neu oedd â chyflyrau meddygol penodol oedd yn cael presgripsiynau am ddim.

Roedd y system yn annheg am nad oedd gan unigolion â chyflyrau penodol fel asthma, clefyd y galon a phwysedd gwaed uchel hawl i gael presgripsiynau am ddim ond roedd gan unigolion â chyflyrau fel diabetes hawl i'w gael.

Mae'r polisi newydd yn sicrhau tegwch i bawb ac mae'r mynediad i feddyginiaethau am ddim yn sicrhau bod unigolion yn cael eu trin yn gynnar a bod achosion o dderbyn cleifion i'r ysbyty yn cael eu hosgoi.

A Framework for Delivering Integrated Health and Social Care For Older Individuals with Complex Needs

Fframwaith ar gyfer Darparu Iechyd a Gofal Cymdeithasol Integredig i Unigolion Hŷn sydd ag Anghenion Cymhleth

Demand for both acute and community care services for older individuals is increasing. The Welsh Government want to ensure that they can deliver effective services that meet their needs.

In order to achieve this, they have developed a framework, which places a strong focus on the involvement of all partners, such as:

  • Local authorities
  • Health
  • Housing
  • Third and independent sector.

The aim is for partners to work together, to delivery high quality integrated services, care and support.

Mae'r galw am wasanaethau gofal acíwt a chymunedol ar gyfer unigolion hŷn yn cynyddu. Mae Llywodraeth Cymru am sicrhau y gall ddarparu gwasanaethau effeithiol sy'n diwallu ei hanghenion.
